Pre-Admission Checklist
What to Bring
Copies of the following documents are needed prior to admission:
- Medicare Card - Part A & B
- Medicare Card - Part D
- Insurance Card
- Medicaid Card (if applicable)
- Social Security Card
- Durable Power of Attorney / Guardianship Document
- Advance Directives / Living Will
We also recommend that you bring 5-7 days' supply of comfortable, seasonally appropriate clothing marked with your name, appropriate non-skid footwear, and any personal toiletries you use. Bring items that can be laundered (not dry-cleaned).
We also encourage you to bring personal select belongings such as pictures, photo albums, writing supplies, knick-knacks, pillows, throws... anything that will help you feel at home. Valuables should be left at home or secured in our facility safe. You may also have a phone installed in your room for private use. We can assist you in making arrangements with the local phone company.
